Book, View Records, Get Help
Use this guide before you open the NHS App, request a GP appointment, order repeat medicine, check test results, ask for health-record access, cancel an appointment, register with the surgery, or travel to 67 Washway Road.
Key point: the NHS App is useful, but it does not replace every GP route. Washway Road also uses an appointment request form, reception, phone support, NHS account functions, pharmacy support and formal records forms.

Repeat Prescriptions: NHS App, 3 Working Days and Pharmacy Choice
Easiest route
Use your NHS account through the NHS website or NHS App. It shows your repeat medicine and dosage so you can choose what you need.
Paper option
You can bring the paper form to the surgery when open. The practice does not take repeat requests by phone or email.
Collection timing
You can usually collect from the pharmacy 3 working days after ordering.
| Prescription issue | Best fix | Prepare |
|---|---|---|
| Need a nominated pharmacy | Choose or change pharmacy in NHS App, at GP practice or at a pharmacy that accepts repeats. | Pharmacy name, address and timing before next order. |
| Medicine question | Ask local pharmacist first; phone practice pharmacist after 10am if you need GP-surgery help. | Medicine name, strength, side effect or question. |
| Overdue medicine review | Respond to self-book invite and arrange review. | BP readings, blood tests, symptoms or side effects if relevant. |
| Old medicines | Return to the pharmacy or bring to surgery. | Do not put medicines in household bin or flush them. |
| Weekend/bank holiday risk | Order early. | Allow working-day processing plus pharmacy stock time. |
GP Records: NHS App vs Formal Access Request
NHS App record
Use NHS App/NHS account to see parts of your GP record, test results, vaccination record and NHS number where enabled.
Formal access form
Use the access-to-health-records form when you need specific data, episodes, dates, or a formal Subject Access style request.
Not urgent help
The official records form includes a “not for urgent medical help” confirmation. Use 111/999 or GP route if care is needed now.
| Records task | What the official form asks for | Speed-up tip |
|---|---|---|
| Request your own data | Name, NHS number, date of birth, address, contact number and request details. | Give exact dates, conditions, documents or episodes. |
| Request for another person | Relationship and legal/consent basis such as signed authority, parental responsibility, LPA or other reason. | Bring or attach proof of authority if requested. |
| Correct, restrict or erase data | Form asks for detail about data requested or action such as restriction, erasure, rectification or objection. | Be specific; vague “send everything” requests are slower to process. |
| Urgent clinical issue | Records form is not the right route. | Use appointment request, phone, 111 or 999 depending on urgency. |
Proxy Access: Carer, Parent or Family NHS App Help
What proxy access does
A patient can let another person use selected online services on their behalf, such as appointments, repeat prescriptions and viewing GP record.
Do not share passwords
The practice warns that sharing your own username/password makes it hard to tell who accessed the record and can create safety risks.
ID may be needed
Proxy forms may require identity checks for both patient and applicant, such as photo ID and proof of residence where appropriate.
Test Results: NHS App, 7-Day Timing and MyMFT Difference
| Result need | Official detail | Best action |
|---|---|---|
| Routine result | If results show more tests or treatment are needed, the practice will contact you. | Check NHS App after doctor review. |
| No NHS App access | You can phone after 11am. | Call outside the urgent morning pressure window. |
| Standard investigation | Results are usually available 7 days after investigation. | Do not chase too early unless symptoms are worsening. |
| X-ray or MRI | Can take 4-6 weeks to be reported. | Ask the requesting clinician if urgent symptoms develop. |
| Hospital result in MyMFT | MyMFT holds hospital-care information and is not linked to NHS App or the practice EMIS record. | Check NHS App to see if the GP practice has received/coded the hospital information before contacting. |
| Want explanation | Use the test-results request form if you need to discuss results. | State the test name/date and your question. |
Sick Notes: 7 Days or Less vs More Than 7 Days
7 days or less
Your employer should not ask for GP medical evidence. Use self-certification when you return to work.
More than 7 days
Request a sick note using the sick-note request form or by phoning the surgery when open.
After hospital admission
If the fit note follows hospital admission, the hospital/consultant can issue it. Contact the consultant where possible.
Home Visits: Housebound Patients and 8am Call Tip
Who it is for
If you are housebound and need an appointment, the practice says it will do a home visit and phone first to understand what you need.
When to request
It is helpful to phone as soon after 8am as possible on 0161 962 4354.
Online option
You can also fill in an online form. Use 111/999 instead if symptoms are urgent or life-threatening.
