RapidHealth / Smart Triage: What Hathaway Uses in 2026
Hathaway says Smart Triage is made by Rapid Health and launched from 26 January 2026. It is designed to route patients to the most appropriate service or team and reduce phone waiting.
Use it for
GP appointments, GP-team messages, administrative requests, letters, fit notes and appointment booking routes.
Email matters
To self-book appointments through Smart Triage, the practice says you must have a valid email address on your patient record.
Children under 16
You can submit a child’s medical request, but you may not always be able to self-book; the practice may review and respond.

Start with triage
GP appointments can only be made through the triage system. Use RapidHealth, or phone if you cannot use the online link.
Stay available
The practice says to make sure you are available for a phone call or SMS on the day you submit the triage form.
Use one problem per GP slot
GP appointments are 10 minutes and normally for one problem. Multiple unrelated issues may need separate forms or appointments.
Use the right non-GP clinic
Some routine non-GP appointments can be made by phone: blood tests, smears, pill checks, asthma/COPD reviews, hypertension and chronic-condition checks.
Arrive early
For face-to-face appointments, arrive about 5 minutes before the appointment to book in.
Ask for extra support early
Tell the practice if you need translation, hearing support, help understanding or remembering information, or longer appointment time.

Order repeats through NHS App or SystmOnline. Hathaway says this is the easiest and preferred method.

Fit Notes, Sick Notes, Letters and Non-NHS Work
Fit notes
Use the triage/admin route for fit-note requests. If hospital treated you, the hospital may need to provide the note for that episode.
Seven days or less
If you are off sick for 7 days or less, employers should not normally ask for GP medical evidence.
Private work
Private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness-to-travel certificates and private prescriptions may be non-NHS chargeable work.
Home Visits: Who Qualifies and When to Call
Who qualifies
You may only request a home visit if housebound or too ill to visit the practice. The GP decides if the condition requires a visit and how urgent it is.
When to call
The appointments page says phone before 10:30 if you think you may need a home visit.
Afternoon cut-off note
A later patient notice says the cut-off for home visits is 12pm; call early because afternoon visiting is restricted.
New Patient Registration: Boundary, Forms and Timing
Check boundary first
Hathaway welcomes registration requests from patients living in or moving to the practice area, but says you must be within the practice boundary.
Do not register out of area
The registration page says out-of-area applications will not be accepted.
Use online registration if suitable
The practice uses the NHS “Register with a GP Surgery” service. You do not need proof of address, immigration status, ID or NHS number to start that NHS-run process.
Allow processing time
After the registration form is returned, Hathaway says to allow 5-10 working days for processing.
Newborn registration
Parents of newborns are asked to register as soon as possible and no later than 4 weeks of age so the 6-week check and immunisations can happen.
Care home registration
For care home registrations, contact the surgery directly rather than using the standard route.

Why you may not need a GP
Hathaway’s services page says staff may ask questions to guide you to the most appropriate care. The right clinician may be a nurse, specialist paramedic, HCA, physiotherapist or living-well/social prescribing team.
