Calling reception no longer skips the GP triage queue
Fremington Medical Centre changed its GP appointment system from 29 January 2025. Clinical requests are now reviewed by a GP before an appointment is allocated, so reception does not simply hand out GP slots when you call.
The useful part is that patients who cannot manage the online form are not excluded: reception can complete the same clinical form with you. Online, phone and in-person clinical requests ultimately enter the same triage queue.
GP Appointments Start With Clinical Triage
Fremington’s latest clinical-triage page says the current model began on 29 January 2025. The aim is to allocate care by medical need rather than simply by whoever reaches reception first.
Submit a clinical request
The preferred route is the online consultation form available from Fremington’s official appointment page or through supported NHS App access.
If you cannot complete it online, contact the practice
You can call or go to reception. The Patient Services Team can complete the same clinical form with you if your circumstances make online submission difficult.
A GP reviews the clinical request
Reception cannot directly allocate a GP appointment before the clinical form has been reviewed. A GP assesses the need and decides the appropriate timeframe or alternative service.
The Patient Services Team contacts you
Fremington says the response can arrive by telephone, text message or a self-bookable appointment link.
The outcome may not always be a GP appointment
Depending on the problem, the practice may arrange same-day care, a routine appointment with an appropriate clinician or referral to another service such as a pharmacy.
Our Okehampton Medical Centre online GP triage guide gives another Devon example of why the live practice triage process matters more than simply searching for “next GP slot”.
For Same-Day Need, Send the Request Early Where Possible
Fremington’s latest triage guidance asks patients to submit requests before 1:00pm where possible. The practice says requests received before that point can be reviewed the same day and that its target is to respond to all clinical requests within one working day.
FORM Send the request
Use the live practice form or ask staff to complete it with you.
DETAIL Explain the problem
Give enough information for safe prioritisation.
GP GP triage
Clinical need determines the route and timeframe.
PLAN Follow the response
Same-day, routine or another service depending on the assessment.
If You Have Two Separate Health Problems, Submit Two Forms
Fremington’s appointment and clinical-triage guidance specifically says that patients with two health issues should complete a separate form for each problem.
Each problem can have a different urgency, clinician and safest next step. One long mixed request can make triage less clear.
After submitting, Fremington asks patients not to call Patient Services simply to check whether the form arrived.
Official Pages Currently Show Different Online-Form Windows
During this audit, three official Fremington pages displayed slightly different opening windows for online requests. Rather than hiding the inconsistency, it is more useful to show it.
| Official page | Published online window | How to use this safely |
|---|---|---|
| Clinical Triage | 7:30am–6:30pm working days | This is the newest detailed triage guidance found in the audit. |
| Appointments | 7:00am–5:30pm working days | Its main process remains the same: submit, then GP triage. |
| AccuRx | 8:00am–6:00pm Monday-Friday | The page predates the latest June 2026 clinical-triage update. |
Core Surgery and Telephone Hours
Fremington currently displays core opening and telephone access from 8:00am to 6:30pm Monday to Friday.
| Day | Core opening | Extra access |
|---|---|---|
| Monday | 8:00am–6:30pm | 6:30pm–8:00pm pre-booked enhanced-access appointments |
| Tuesday | 8:00am–6:30pm | Normal core day |
| Wednesday | 8:00am–6:30pm | 7:30am–8:00am pre-booked enhanced-access appointments |
| Thursday | 8:00am–6:30pm | 6:30pm–8:00pm pre-booked enhanced-access appointments |
| Friday | 8:00am–6:30pm | Normal core day |
| Saturday | Closed | Pre-bookable extended-access appointments may be offered through the wider local service |
| Sunday | Closed | Use the appropriate out-of-hours route if urgent |
Some Evening and Weekend Appointments May Be at Another North Devon GP Practice
Fremington also participates in a wider Extended Access Services alliance. The practice says additional appointments can be available across GP surgeries in Barnstaple, Bideford, Hartland, Northam and Torrington.
Early appointments
The wider service includes some appointment availability from 8:00am to 8:30am.
Evening access
Extended Access can include evening appointments up to 8:30pm at participating sites.
Weekend access
Weekend appointments may also be available and can involve a GP, nurse or healthcare assistant depending on the need.

When Fremington Is Closed
The practice says patients who need urgent medical advice when it is closed should use NHS 111.
| Situation | Route | Practical meaning |
|---|---|---|
| Life-threatening emergency | 999 | Do not wait for GP triage or the surgery to reopen. |
| Urgent medical problem when closed | NHS 111 | For children under 5, Fremington says to call 111. People aged 5 and over can also use 111 online. |
| Pre-booked extended-access appointment | Follow booking confirmation | Attend the time and location shown; it may be another practice. |
| Routine admin or repeat prescription | Normal practice/online route | Do not use urgent out-of-hours care for a task that can safely wait. |
Home Visit Requests Should Be Sent Before 11am Where Possible
Fremington asks patients to complete the online form before 11:00am where possible if they need a home visit.
The practice says home visits can be requested when somebody is housebound or too ill to attend. The GP decides whether a home visit is clinically required and how urgently it is needed.
Repeat Prescriptions: Allow Five Working Days
Fremington’s repeat-prescription page currently asks patients to allow 5 working days for processing and recommends ordering when you have fewer than seven days of medication remaining.
Use the practice online form for a repeat prescription request or prescription query where appropriate.
Request medicines that appear on your repeat list and check your nominated pharmacy.
Fremington also accepts SystmOnline requests and traditional repeat ordering slips through the surgery or pharmacy.
Our Bideford Medical Centre NHS App and records guide provides another nearby North Devon example of managing prescriptions and GP online services without relying only on reception calls.
AccuRx, NHS App and SystmOnline Do Different Jobs
Fremington currently publishes all three online-service routes. They overlap, but they are not identical.
AccuRx
Used for clinical and administrative online forms, questions, prescription-related requests, fit notes and other practice contact.
NHS App
Fremington says the app can support appointments, repeat prescriptions, NHS-number access and GP record features where enabled.
SystmOnline
The practice says SystmOnline can be used for selected routine appointments, repeats, results and record access.
Fremington Is Currently Accepting New Patients
The current NHS GP profile says Fremington Medical Centre is accepting new patients. The practice asks people living in its catchment area to use its registration process and also publishes separate guidance for out-of-area and temporary registrations.
Check the practice catchment
Fremington provides a catchment-area checker on its patient information pages.
Open the official registration route
Use Fremington’s new-patient registration page rather than sending private details to a third-party directory.
Complete the forms requested by the practice
Follow the instructions shown in the current registration process and provide accurate contact information.
Understand named-GP allocation
Fremington says new patients are assigned a named accountable GP, but this does not prevent you from seeing another doctor or nurse when appropriate.
If your address is outside Fremington’s catchment and you want to compare other surgeries, use our NHS GP and medical-centre finder guide and then verify the boundary on the chosen practice’s own website.
Fremington Is a Training Practice With a Wider Clinical Team
Fremington describes itself as a training practice. This means some consultations may be with a GP registrar or another qualified clinician working under the practice’s training arrangements.
GPs and registrars
The current team page lists GP partners, salaried/retainer doctors and doctors undertaking GP or foundation training.
Nurse practitioners
Advanced Nurse Practitioners and a Minor Illness Nurse are part of the clinical team.
Other professionals
Practice nurses, healthcare assistants, physiotherapists and community staff contribute to patient care.
This matters because GP-led triage may deliberately direct you to another professional if that person is better suited to the problem.
Access, Bus Routes and the Tarka Trail
Fremington’s contact page was updated in March 2026 and gives useful local arrival information.
Disabled access
The practice lists ramp access, disabled toilets on the ground and first floors and a lift to the first floor.
Bus routes
Fremington lists Stagecoach routes 21, 21A and 5B as serving the area.
Walking and cycling
The practice specifically highlights the Tarka Trail for patients who can walk or cycle to the surgery.

Common Fremington Problems and the Useful Next Step
| Problem | Why it happens | What to do |
|---|---|---|
| Reception will not directly book a GP appointment | Fremington uses GP-led clinical triage | Complete the clinical form online or ask staff to complete it with you. |
| You called instead of going online | Phone requests still enter the same triage queue | Give staff the information needed to complete the same form. |
| You submitted a form and want to confirm receipt | The practice asks patients not to phone merely to confirm submission | Watch for text, telephone or self-booking contact unless your condition changes. |
| You have two separate symptoms | Each problem needs its own triage assessment | Complete a separate form for each health issue. |
| Online form appears closed earlier or later than expected | Current official pages display different form-hour windows | Use the live website button; submit before 1pm where possible for same-day review. |
| You are offered an evening or weekend appointment elsewhere | Fremington participates in a North Devon extended-access alliance | Check the exact surgery address in the booking confirmation. |
| Repeat prescription has not appeared quickly | Fremington asks patients to allow five working days | Check submission and nominated pharmacy details before sending duplicates. |
| Urgent problem when the practice is closed | Normal GP triage is not the out-of-hours route | Use NHS 111 if urgent and not life-threatening; call 999 for an emergency. |

A GP reviews the request before the appointment timeframe or alternative service is decided.
