Find the Right Doctor and NHS Route Before You Travel
Chapel Medical Centre is an NHS GP practice inside the Upton Hospital site in Slough. The current practice website publishes a substantially newer doctor list than the old CQC inspection report, so this guide uses the practice’s September 2026 team page for current names.
It also matters to separate the GP surgery from Slough Urgent Care Centre. Old inspection material described a walk-in service at Upton Hospital, but today’s urgent-care centre is at Priors Close.
Doctors Currently Published by Chapel Medical Centre
The practice’s own “Our Staff” page was reviewed on 9 September 2026. It currently lists the following clinicians under Doctors.
A name on the staff page does not mean that clinician is available every day or for every appointment type. Tell reception if you have a preferred doctor, but allow the practice to offer another appropriate clinician when needed.
Your Appointment May Be With Someone Other Than a GP
Chapel’s current staff page shows a broader clinical team. That matters because modern primary care often routes a patient to the professional best suited to the task rather than automatically booking a doctor.
Physician Associate
The current practice page lists Maira Ahmad.
Nursing team
The practice currently lists Elisa Deladia as lead, with Babara Asante, Stephanie Lodge, Kathleen Evangelista and Vera Filipe.
Nurse associate & HCA
Portia Alcedo is listed under Nurse Associates, and Shadeke Richardson under HCA.
Clinical pharmacist
The current team page lists Kiran Ghatore as Clinical Pharmacist.
The practice also lists Dr Sufian Jabbar as Clinical Lead and Suzy Simmons as Primary Care Manager.
Opening Hours: What Is Verified and What Needs Rechecking
CQC’s published inspection material and older practice literature both record the GP surgery’s core weekday opening pattern as 8:00am to 6:30pm Monday to Friday.
| Day | Published core-hours baseline | Current-use note |
|---|---|---|
| Monday | 8:00am–6:30pm | Check live practice information before travel. |
| Tuesday | 8:00am–6:30pm | Training closures can shorten the day. |
| Wednesday | 8:00am–6:30pm | Check current notices before a time-sensitive visit. |
| Thursday | 8:00am–6:30pm | Check the current appointment/opening route. |
| Friday | 8:00am–6:30pm | Out-of-hours care begins after the core service closes. |
| Saturday | No ordinary core GP hours verified | Use pre-arranged extended-access or urgent-care routes as directed. |
| Sunday | No ordinary core GP hours verified | Use the appropriate local urgent/out-of-hours service. |

Upcoming Staff-Training Closures
The practice homepage was reviewed on 23 September 2026 and currently publishes the following planned staff-training closures.
20 October 2026
Surgery closed from 12:30pm until 8:00am the next day.
18 November 2026
Practice homepage lists another planned training closure from 12:30pm until 8:00am the following day.
19 January 2027
The next published training date after 2026 is also shown on the current homepage.
How Chapel Handles Same-Day and Routine Appointments
Chapel’s appointment information says the practice offers urgent same-day telephone appointments, urgent appointments and routine appointments.
More importantly, the current September 2026 homepage says patients requesting an urgent clinical appointment or review should contact the practice by telephone.
For an urgent clinical request, phone reception first.
Explain what help you need so the team can direct the request.
Same-day primary care may be at Chapel or another service.
Attend or answer the telephone according to the booking offered.
Current homepage: telephone the practice for an urgent clinical appointment or review.
Use the practice’s current booking route or supported online services if already set up.
Online Appointments, Prescriptions and Record Access
Chapel’s current homepage still promotes Access Online Services and the NHS App. Its online-services information describes support for tasks including appointment booking/cancellation, repeat prescriptions and access to parts of the GP record.
Appointments
Registered patients with online-service access can use supported appointment functions where currently enabled.
Repeat prescriptions
The practice publishes an online repeat-prescription route and also supports electronic prescription collection.
GP-record access
Chapel states that supported online services can provide access to parts of the patient’s medical record.
Another useful comparison is our Acle Medical Centre online GP and NHS booking guide , which also separates online patient-account functions from the route used to request clinical help.
Current Slough Urgent Care Is at Priors Close — Not Upton Hospital
Chapel’s current homepage directs patients to Slough Urgent Care Centre at Priors Close for appropriate same-day minor-illness care.
Current location
Priors Close, Slough, SL1 2BQ.
Published hours
The current Chapel homepage says the centre operates 8am–8pm Monday to Sunday.
Type of problem
Chapel describes it as an option for minor illnesses needing same-day care, such as chest, skin or urinary tract infections.
Patients may be booked into the centre by their GP practice or NHS 111. Chapel also says that if a patient cannot obtain an appointment through the practice or NHS 111, they can walk in and be given a booked appointment, with the option to wait or return at that time.
Why Older Search Results Can Send You to the Wrong Walk-In Location
CQC’s 2019 inspection report said Chapel Medical Centre was co-located with a walk-in service at Upton Hospital. That statement described the service at the time of the inspection.
Later local reporting records that the previous Upton walk-in centre closed in 2020 and the Priors Close urgent-care service opened in 2023.
Evening and Weekend GP Access
Chapel’s older appointment guidance describes Slough LOCC extended access outside ordinary core GP hours. Because that appointment page itself was last reviewed in 2019, this article does not present its historic evening/weekend timetable as a guaranteed 2026 schedule.
The current practice homepage does, however, make clear that local same-day primary-care appointments can be delivered at the practice or another location.
Registering With Chapel Medical Centre
The practice states that it serves patients living in the Slough area and provides an online new-patient registration form as well as a paper registration option.
Open the current registration page
Complete the new-patient form
Chapel currently provides an online form for adults or children.
Use a paper form if you prefer
The practice also links to a paper NHS registration form.
Set up online access separately if needed
Secure online-service access may require identity verification because it can expose appointment, prescription and medical-record information.
If you are moving within Berkshire or are unsure which surgery covers your address, our NHS GP surgery finder and registration guide explains how to verify a practice before applying.
Repeat Prescriptions and Electronic Pharmacy Collection
Chapel provides an online repeat-prescription request and supports the NHS Electronic Prescription Service (EPS).
Use the current prescription form or your supported online GP/NHS service.
Chapel’s registration/prescription material asks patients to nominate a pharmacy for electronic prescription collection.
Clinics Published by the Practice
Chapel’s clinic information describes several GP-practice services. Some require clinician referral, so they should not be treated as self-bookable simply because they appear on the website.
Musculoskeletal clinic
The practice’s clinic page says MSK clinics are held on Friday mornings and require clinician referral.
Physio assessment
Chapel publishes a physiotherapy assessment clinic and tells patients to contact the surgery about booking.
Minor operations
Suitable patients are referred by a clinician for the practice’s minor-operations service.
Accessibility and Parking
Chapel’s practice-information page lists several access features at the Upton Hospital site.
Wheelchair access
Wheelchair access is published by the practice.
Parking
On-site parking and disabled parking are listed.
Disabled WC
A disabled toilet is listed on site.
Induction loop
An induction loop is available for patients who require it.
The practice also asks patients with impaired sight to tell reception if they need correspondence in large print.
Home Visits Are for Patients Who Cannot Reasonably Attend
Chapel says home visits are intended mainly for genuinely housebound patients or circumstances where attending the surgery is not reasonable.
- Call 03000 24 0001.
- Make the request before 10am where possible.
- The clinician on call will telephone you to discuss the request.
- A doctor may decide a telephone consultation is more appropriate.
- The clinician decides whether a home visit is medically appropriate.
Why Someone Who Arrived After You May Be Called First
Chapel’s current practice-information page says the waiting room is shared with another service, including emergency dental.
Patients are called by the clinician for the service they are due to see, so somebody who entered after you may legitimately be called before you.
Chapel’s Place in Slough Primary Care
NHS workforce information places The Chapel Medical Centre in the Slough LOCC Primary Care Network alongside local practices including Langley Health Centre, Cippenham Surgery and The Orchard Surgery.
This wider network context helps explain why extended-access and same-day appointments can sometimes be delivered away from the patient’s own GP building.
